Before you start Act 6
Act 6 is the final stretch and it does not let up. Before you begin:
- Spend every skill point you have, and your Paragon points too if you have unlocked them.
- Upgrade your Healing Potion at the Alchemist if a new one is ready.
- Replace any weak gear pieces. A single old item can make a big difference in the last fights.
- Empty your bags. There is a lot of loot on the way down.

Act 6: Dance of the Makers
Caldeum
- The Jewel of the East: travel to Caldeum, the capital of Kehjistan. Lilith's forces have the city in their grip.
- The Scouring of Caldeum: fight through the streets with your allies. Stay close to your allies and clear each area before moving on.
- The Walls Shake: find your way into the Imperial Palace and defeat Duriel.
Boss: Duriel, Lord of Pain
Duriel is a giant, maggot-like demon and a classic Diablo boss. He also appears in the endgame in a much harder form, so many online guides describe that version. The campaign fight is simpler.
- He hits very hard in melee. Do not stand in front of him and trade blows.
- Watch for him going underground or charging. When he does, move, and be ready to dodge when he comes back up.
- Clear any small enemies that join in so you have room to move.
- Use your potion early. His big hits come fast.
Into Hell
- Turning the Tide: descend with your allies to the Gates of Hell. This quest starts the final push.
- Essence of Hatred and In Desolation's Wake: push through the Burning Hells. Expect long fights against big groups of demons. Keep moving and do not get surrounded.
- Light Extinguished and The Blind Eye: major story moments happen here. Follow the quest markers and stay with the group.
- What Lies Ahead: reach the Cathedral of Hatred and face Lilith.
The Hell quests have few safe spots. If you need to leave, use a Town Portal and come back. Your progress is kept by the quest log.
Final boss: Lilith
Lilith is a two-phase fight. In the first phase she is Lilith, Creator of Sanctuary. In the second she becomes Lilith, Daughter of Hatred. The second phase is the hard one.
Phase 1:
- She is fast and uses sweeping melee strikes with her wings. Dodge sideways, not backwards, to get clear.
- Many attacks leave dangerous patches on the floor. Move off them right away.
- When her health gets low, she jumps to the middle of the arena, hovers and summons minions. Kill the minions fast. When she lands again, phase 2 begins.
Phase 2:
- She destroys pieces of the platform over time, so the arena keeps getting smaller. Stay near the centre and never fight on the edge.
- There are fewer ways to heal, so play safe. Do not drink your potion until you need it, but do not wait until one hit will kill you either.
- Her stagger bar is harder to fill now. Do not count on stunning her. Just keep steady damage going.
- Save your dodge and any "unstoppable" skill for her big moves, so you can escape crowd control.
If you wipe a few times, that is normal. Lower the difficulty for this one fight if you just want to see the ending. There is no shame in it, and you lose nothing.

Epilogue: From the Wound Spilled
After the final cinematics, the epilogue starts. It is short, has no hard bosses, and sets up the story of Vessel of Hatred.
- Promises: go with Lorath to an old chapel in the desert to meet Neyrelle, who now carries the Soulstone that holds Mephisto.
- A Heavy Burden: search for Neyrelle in the Horadric Vault and read the Runed Letter she left behind.
- Legacy of the Horadrim: meet Lorath at Firebreak Manor in Scosglen and talk to him. This ends the base campaign.
After the epilogue, the Tree of Whispers and the rest of the endgame open up, and your other characters can now skip the campaign. If you own the expansion, the next part of this walkthrough picks up Neyrelle's story.

A big open world and lots of cutscenes: a good gaming headset really brings Diablo IV's sound design to life.
Tips for this part
- Do not rush into Lilith. Clear the path, top off your potion and make sure your skills are off cooldown first.
- Learn the phase change. When Lilith jumps to the middle, stop attacking her and focus her minions.
- Stay central in phase 2. Most deaths come from being near a piece of floor that is about to fall.
- Bring a friend. Co-op makes the Hell quests and both final bosses much easier.
- Do not panic about endgame guides. The campaign versions of Duriel and Lilith are much easier than their endgame forms.
My take
The march into Hell is great fun, and Lilith is one of the best final bosses in the series. The shrinking platform in phase 2 had me sweating on my first run. The epilogue is quiet, but it is worth playing for the hook into the expansion. How many tries did Lilith take you?
FAQ
How do I beat Lilith in Diablo 4?
In phase 1, dodge her wing strikes and floor effects, and kill her minions when she hovers in the middle. In phase 2, stay near the centre because she breaks the platform edges, play safe with healing, and save your dodge for her big moves.
Which quest is the Lilith fight?
The final Act 6 quest, What Lies Ahead, in the Cathedral of Hatred.
Where is Duriel in the campaign?
In the Imperial Palace in Caldeum, during the Act 6 quest The Walls Shake.
What happens after the Diablo 4 campaign?
You play the short epilogue, then the endgame opens: the Tree of Whispers, harder difficulties, dungeons and seasons. If you own Vessel of Hatred, you can start the expansion story next.
